How to Request Disability Services
1. Schedule an appointment to meet with a Disability Services Counselor using the “Request for Registration” form.
2. Support your request for services by providing documentation for your disability using the guidelines set forth in the “Students with Disabilities Preparing for Postsecondary Education: Know Your Rights and Responsibilities” handbook.
3. Follow the outlined sequence to receive Disability Services (PDF).
